If you move house or change
your telephone number, please let us have the new details. This is
very important to enable us to keep in contact with you. If you move
outside of the practice area, in most cases you will need to register
with a new doctor.

WITH PRACTICE?
Any person living permanently
(ie. over six months) within the practice area, may apply to register
at the surgery. Application forms can be obtained from reception.
